Job Location: 601 Elmwood Ave, Rochester, New York, 14642 Schedule: Full time, 40 hours per week, Evening shift (8:00pm–4:00am) Compensation Range: $126,258 – $163,610 annually Responsibilities Provide comprehensive clinical pharmacy services related to a specialized area of practice to optimize safe, effective, and cost‑effective drug therapy. Participate in inter‑disciplinary patient care teams and deliver pharmacotherapeutic and pharmacokinetic consults. Educate patients, providers, and pharmacy colleagues on medication management and best practice guidelines. Lead departmental initiatives, deliver education and teaching for pharmacy colleagues, providers, and trainees at all levels. Contribute to departmental objectives as an integrated member of the pharmacy practice team. Essential Functions Participate on patient care team rounds and provide consultative input on safe and effective drug therapy. Manage established clinical pharmacy programs or services and monitor patient populations for clinical progress. Recommend therapy adjustments, provide patient education, discharge counseling, and authorize and verify orders. Document clinical activities in the medical record and other systems. Collaborate with clinicians and healthcare providers to implement medication‑management programs. Develop global medication‑use management programs, clinical guidelines, and critical pathways. Serve as a clinical mentor, coach, and role model for all pharmacists in the department. Supervise pharmacy technicians to ensure appropriate workflow. Conduct quality assurance, medication‑use evaluation, and research to document program value. Participate in departmental Q/A processes, reporting medication incidents, reconciling medication administration records, and cost‑saving initiatives. Educate pharmacy residents, students (IPPE/APPE), and other health‑professionals through didactic lectures and case studies. Resolve patient‑care problems, communicate progress clearly, and involve supervisors when necessary. Maintain active membership in local, regional, and national professional organizations and present educational or research results at conferences. Provide continuing professional education to peers and serve on committees or task forces. Perform other duties as assigned. Minimum Education & Experience Doctor of Pharmacy degree required. Specialty residence training preferred. Equivalent combination of education and experience acceptable. New graduate or clinical specialist in the first 3–5 years of practice preferred. Licenses & Certifications New York State Pharmacy License upon hire required. Board certification in the most relevant specialty area within 2 years required.
