As a Critical Access Hospital serving East Central Illinois and Northwest Indiana, IMH offers physicians the opportunity to practice broad-spectrum primary care while working closely with specialists, hospital providers, nursing homes, and community partners. Position Highlights Outpatient primary care practice in a Rural Health Clinic setting Opportunity to build long-term relationships with patients and families Collaborate closely with experienced Nurse Practitioners, specialists, hospital providers, and care management teams Participate in population health initiatives, including Annual Wellness Visits, chronic disease management, and preventive care programs Opportunity for nursing home rounding. Access to on-site laboratory, imaging, specialty clinics, rehabilitation, and hospital services Supportive leadership team committed to provider engagement and reducing administrative burden Competitive compensation and benefits package What We're Looking For MD or DO licensed (or eligible for licensure) in Illinois Board Certified or Board Eligible in Family Medicine or Internal Medicine Commitment to providing high-quality, patient-centered care Strong communication and team collaboration skills Interest in rural healthcare and serving underserved communities Willingness to participate in quality improvement and population health initiatives Why IMH?
