Physical Therapist

Overview Nucleus Healthcare is seeking an Outpatient Physical Therapist in Oklahoma City, OK. Shift: Day - M-F 8 Hours. Length: 13 Week Contract. Location: Oklahoma City, OK. Responsibilities Evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with patient needs and physician directions. Delegates and directs all discipline‑specific therapy services for assigned patients. Designs functional treatment programs to facilitate effective return to community activities. Educates patients, families, students, coworkers, and self. Performs other professional duties related to patient care, discipline‑specific service development, and program delivery. Completes comprehensive evaluations of assigned patients, developing treatment plans and goals aimed at discharge within the established length of stay. Collaborates with the interdisciplinary team to remove discharge barriers, including equipment trialing, training, and continuing services. Manages caseload flexibility, coverage, and clear communication of the plan of care. Leads individual, concurrent, and group treatment sessions based on patient needs. Documents medical records accurately: daily entries, goal writing, functional quality indicators, letters of medical necessity, discharge notes, and charge entry using approved formats. Ensures care is delivered under signed, valid physician orders and within insurance approval requirements. Participates in student programs, providing supervision, learning objectives, and evaluations. Creates a professional development plan, pursuing continuing education, self‑study, and professional activities. Advocates for patients and professionals through lobbying, health fairs, community events, memberships, and fundraising. Adheres to state practice acts, maintains licensure, and fulfills continuing education mandates. Engages in performance improvement initiatives to enhance service quality. Provides feedback during staff meetings, focus groups, or to supervisors. Qualifications BLS/HCP required. Preferred: related experience in clinical applications. Degree in Physical Therapy from an accredited school. 1+ year of outpatient clinical experience. CPR certification.
