Position works as a provider member within the HBMC interdisciplinary team in the direct delivery of home‑based medical care of a population within an assigned geography. Dedicated to improving health and well‑being, this role collaborates with PCP, HBMC interdisciplinary teammates, consultants and other health professionals to develop and carry out patients' goals of care throughout the patient journey. Primary Responsibilities Perform an initial comprehensive assessment of all newly enrolled patients and provide ongoing care thereafter. Develop a patient management care plan upon enrollment and update it as needed when changes in condition warrant or following hospitalization. Meet with patients and/or their legal representatives to review care plans; involve the PCP or supervising physician when applicable. Be comfortable with basic nursing procedures (IV placement, blood draws, injections, foley catheters, nasal packing, wound care, etc.) and attend new hire and annual skills check/training. Prescribe appropriate diagnostics and interventions to avoid unnecessary acute admissions. Complete follow‑up and post‑discharge assessments according to documented SOP. Consult with hospital, emergency or post‑acute clinical team following patient transfer. Educate patients/legal representatives on disease processes and effective management, and promote wellness. Actively participate in ongoing patient‑care and clinical excellence meetings. Implement HEDIS measure campaigns and other quality initiatives to ensure high standards of care and improve care management. Stay current on medical and nursing research, technology, and related issues via continuing education, professional meetings, and journal reviews. Practice in accordance with state laws and regulations governing advanced practice nursing. Required Qualifications Completion of an accredited Nurse Practitioner program. Current state NP license or ability to obtain by start date. Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or ability to obtain national certification and/or NP license in the state of assignment by start date. Current active DEA licensure/prescriptive authority or ability to obtain post‑hire, per state regulations (unless prohibited in state of practice). Reliable transportation; if driving, must comply with Optum Motor Vehicle Safety policy. Preferred Qualifications 2+ years of clinical NP experience in IM, Geriatrics, Primary Care, ED, Urgent Care, Home Health Care visits or similar setting. Excellent administrative and organizational skills and ability to effectively communicate with seniors and their families. Computer literate and able to navigate the internet. Compensation ranges from $109,500 to $164,000. Total cash compensation includes base pay and bonus and is based on local labor markets, education, experience, and may increase over time based on productivity and performance.
