*Position Summary*
We are seeking a PRN Occupational Therapist to provide in-home therapy services to patients throughout Northwest Indiana. This role offers a flexible schedule, making it an excellent fit for clinicians looking for supplemental hours or greater control over their caseload.

*Responsibilities*

Conduct occupational therapy evaluations and develop individualized treatment plans based on patient needs and physician orders

Provide skilled OT interventions in patients' homes to improve function, safety, and independence with daily activities

Document evaluations, progress notes, and treatment plans in compliance with agency and regulatory standards

Communicate and collaborate with physicians, nursing staff, and other members of the care team

Educate patients and caregivers on home safety, adaptive equipment, and self-management strategies

Monitor patient progress and adjust treatment plans as needed

*Qualifications*

Current, unencumbered Occupational Therapist license in the state of Indiana

Degree from an accredited Occupational Therapy program; NBCOT certification required

Home health experience preferred but not required

Valid driver's license, reliable transportation, and current auto insurance

Strong communication and time-management skills; ability to work independently
