Registered Nurse - Hospice

Role Overview: The Registered Nurse Case Manager is responsible for delivering comprehensive hospice care by developing and implementing patient-specific Plans of Care, collaborating with interdisciplinary teams, and evaluating patient needs regularly. Key Responsibilities Conduct initial nursing evaluations to determine hospice eligibility within 48 hours of referral. Assess and reassess patients' physical, psychosocial, and environmental needs at least every 15 days. Develop, coordinate, and document the care plan, goals, interventions, and patient responses. Collaborate with patients, families, physicians, and team members to ensure quality care. Educate and supervise patients and families on self-care techniques when appropriate. Maintain accurate clinical documentation regarding patient conditions. Qualifications & Skills Current, unrestricted RN license in the practice state. CPR certification or ability to obtain within 90 days. Valid driver’s license, reliable vehicle or access to transportation. Ability to work effectively in diverse home settings regardless of background or circumstances. Minimum of 1+ years of clinical experience (state-specific requirements may vary).
