Job Requirements:
Active Vermont RN License
Minimum 3 year of recent Home health experience
Strong assessment, communication, and care coordination skills
Experience with Homecare Homebase (HCHB) EMR is preferred

Responsibilities:
Perform patient admissions, comprehensive assessments, and ongoing evaluations in the home setting.
Develop and implement individualized plans of care for patients of all ages.
Administer medications, treatments, and skilled nursing interventions.
Educate patients and caregivers on disease management, medications, and home care.
Collaborate with physicians and interdisciplinary home health team members to coordinate care.
Accurately document patient visits and care plans using Homecare Homebase (HCHB).
Monitor patient progress and adjust care plans to support optimal health outcomes.
