Job Summary Seeking an experienced Physical Therapist (PT) to provide rehabilitation services in a Skilled Nursing Facility (SNF) setting. The PT will work closely with residents and interdisciplinary teams to improve mobility, functional independence, and quality of life. Must-Have Skills Active Massachusetts Physical Therapist License Strong SNF / Long-Term Care experience (required) Excellent patient assessment and treatment planning skills Knowledge of Medicare and rehabilitation documentation standards Strong communication and teamwork abilities Experience Required Minimum 3 years PT experience required Skilled Nursing / LTC experience required Key Responsibilities Evaluate residents and develop individualized treatment plans Deliver therapy to improve mobility, strength, and ADLs Monitor patient progress and adjust treatment plans Collaborate with physicians and interdisciplinary teams Educate residents and caregivers on therapy goals and exercises Maintain accurate EMR documentation and compliance with reimbursement guidelines Supervise PTA staff and support team-based care Position Details Location: Worcester, MA 01610 Contract: 13-week, 40 hours per week, 8-hour shifts, Day Shift.
