Registered Nurse (RN) – Home Health/Hospice Case Management

Seeking a WI-licensed RN for a Monday–Friday, 8 AM–5 PM home care position with approximately 5 visits per day . Candidates should have wound care experience, CPR/BLS certification, a valid driver’s license, reliable vehicle, and car insurance .

Key requirements:

• RN license in Wisconsin
• Home Health/Hospice and/or case management experience
• Wound care experience
• HCHB experience preferred; MatrixCare/other EMR acceptable
• Strong care coordination, documentation, patient advocacy, and family communication skills
• Elder abuse, HIPAA, infection control, and OSHA training required
• Mileage reimbursement: $0.43/mile
• Service areas include Fond du Lac, Dodge, Columbia, and Winnebago counties , with additional locations in Brookfield, Jackson, Waterford, and Sheboygan

Hospice or HCHB experience gaps may require employer-provided training before the full bill rate applies.
