Key Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans
- Administer therapeutic exercises, manual therapy, and other physical therapy interventions
- Monitor and document patient progress throughout the course of treatment
- Collaborate with interdisciplinary care team to ensure coordinated patient care
- Educate patients and families on home exercise programs and safe functional mobility
Required Qualifications
- Active Physical Therapist (PT) license in the state of Wyoming or ability to obtain prior to start
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Experience working in a skilled nursing or rehabilitation care setting
Preferred Experience / Skills
- Prior experience in a long-term care or post-acute rehabilitation environment
- Familiarity with Medicare documentation and compliance requirements
- Strong communication and patient-centered care skills
