Job Description
Title
Hospice RN Case Manager

Schedule
Monday-Friday, 8:00 AM - 4:30 PM

Assignment Details
Visits/day: ~5
Patient load: ~15-18 patients
Weekend requirement: every other weekend (rotation)
Guaranteed hours: 3 shifts per 13 weeks

Requirements
- Minimum clinical experience: 1 year
- Required experience: Hospice RN / RN Case Manager experience
- Certifications: BLS required; ACLS required by compliance deadline
- High-acuity experience preferred: wound VACs, Pleurex drains, peritoneal drains, trach care
- Strong computer skills required
- License: IL license required at submittal; MO may be submitted without license (must obtain state-specific license)

Roles & Responsibilities
- Conduct patient admissions and triage effectively
- Perform routine visits, ensuring comprehensive care and assessment
- Manage medications and durable medical equipment for patients
- Provide wound care and supply management as needed
- Educate patients and families on care plans and equipment use
- Collaborate with interdisciplinary teams for optimal patient outcomes

Compliance / Notes
- COVID-19 vaccine not required; Flu vaccine required (exemptions allowed)
- Facial hair must allow proper fit for testing
- Documentation due by the set compliance deadline
- Modules must be completed prior to start date
