Bilingual Lead Care Manager

Manage an assigned caseload of adult and pediatric members Maintain a caseload of up to 50 members and conduct monthly in-person visits Conduct comprehensive assessments of members’ physical, emotional, and social needs Develop and tailor individualized care plans based on assessment findings, medical history, preferences, specific needs, and goals Coordinate communication among healthcare providers, social workers, therapists, and other care-team members Collaborate with medical doctors, clinical consultants, housing navigators, and leaders to make member-specific recommendations Monitor member progress and update care plans according to policy and compliance requirements Ensure prescribed treatments and interventions are followed and communicate significant changes to PCPs and specialty care providers Provide positive member/client service through telephone and in-person support Maintain accurate records of assessments, care plans, and member interactions Ensure compliance with relevant regulations and standards Complete required documentation accurately, timely, and according to company standards Provide leaders with case progress updates periodically or as required Advocate for members by helping them navigate healthcare, understand treatment options, and access services Educate members and families about health topics, treatment options, and self-care Connect members with community resources, support services, and programs such as housing assistance, financial aid, and counseling Plan and coordinate discharge processes from hospitals or long-term care facilities Participate in training new employees Perform other duties assigned or required by departmental policy Requirements Fluent in English, written and verbal Bilingual in Spanish Ability to communicate clearly in person, by phone, and electronically Adequate hearing and vision, with corrective devices if necessary, to conduct assessments and documentation Ability to identify problems and use logic and related information to develop and implement solutions Excellent organizational skills and attention to detail Commitment to maintaining patient confidentiality and adhering to ethical standards in healthcare practice Ability to lift, carry, push, or pull up to 20–25 pounds Ability to climb stairs or navigate uneven terrain in community and home environments Ability to bend and reach and conduct in-person visits in non-traditional environments Ability to remain in a stationary position Ability to move around the office and/or travel throughout the community Ability to operate a vehicle and travel to meet members around the community; attend meetings and events as required or requested Ability to maintain professionalism and safety in diverse environments Ability to work independently and complete assignments within established policies and procedures Frequent use of computers, keyboards, and handheld/mobile devices Ability to type for extended periods Competent with computers, email, virtual platforms, and Microsoft Office-based programs Diploma/GED required Minimum 1+ years of experience in healthcare, social services, or care coordination Current and valid driver’s license with a clean driving record Reliable transportation and proof of auto insurance Current BLS certification from the American Heart Association upon start date Distraction-free home workspace with a secure internet connection Core Competencies Demonstrates expertise in managing caseloads and conducting comprehensive assessments to develop individualized care plans.
