Eaton County Health and Rehabilitation ServicesEaton GardensWork Schedule: Monday - Friday, occasional weekends to support admissions Position: Social WorkerThe Social Worker is responsible for resident assessment, care management, care planning, addressing psychosocial concerns, and may serve on the Behavior, Pre-Care Plan, and Care Plan committees.  Principle Duties and Responsibilities:The social worker will perform the following duties listed and others as required to improve the quality of life for residents. Duties and responsibilities may be added or modified at any time. Responsible for exercising discretion and independent judgment to assist the management of the Social Services DepartmentWill encourage residents through counseling and other interventions to achieve their highest level of psychosocial interactionWill implement person-centered care plans for residents and will monitor and follow-up on directives as outlined in the resident's plan of careCounsel residents/families regarding psychosocial, adjustment, and behavioral issues.Participate in Behavior Management Team meetings to monitor resident behaviors and psychotropic medication useParticipate in interdisciplinary care conferences with residents and familiesAssist with resident discharge planningPerforms other duties as required. Working Conditions:Nursing home setting. Lifting up to 25 lbs. when needed. Prolonged standing or walking, bends, lifts, carries, pushes and pulls various equipment and residents throughout the work day. Extended, unusual, or mandated working hours when required.Qualifications:Ability to give instructions, organize time and complete tasks with minimal supervision. Ability to manage multiple requests and projects simultaneously. Excellent written, verbal and interpersonal skill necessary. Experience:A broad knowledge of social work skills and the ability to apply necessary skills and techniques in a medical setting is preferred. Long-term care social work experience preferred. Experience working with PointClickCare software a plus. Education:Masters in Social Work required
