We serve the most vulnerable seniors with better quality and compassion in a value-based model.The Case Manager, RN directly guides participant care as a crucial voice on the interdisciplinary team (IDT). Reporting to clinical leadership, the Case Manager, RN delivers and oversees all aspects of care, including conducting required PACE assessments, developing and following personalized participant care plans, and supporting transitions of care coordination.This role is different because Case Manager, RNs at WelbeHealth:Work a consistent Monday through Friday schedule – no weekends and no on callBuild relationships with participants rather than providing short-term careCareer growth opportunitiesWe care about our team members. That’s why we offer:Medical insurance coverage (Medical, Dental, Vision)Work/life balance - We mean it! 17 days of personal time off (PTO), 12 holidays observed annually, and 6 sick days401K savings + matchComprehensive compensation package including base pay and bonusAnd additional benefits!On the day-to-day, you will:Conduct clinic nursing assessments to determine participant needs, preferences, and goals, and actively participate in IDT meetings to develop participant care plansDeliver and document nursing interventions as agreed upon in participants’ care plans, including but not limited to maintaining a safe environment, promptly responding to physician orders, and correctly administering medicationsCollaborate with the WelbeHealth PACE Physician and other members of the IDT in coordinating participant care needs, as well as communicate with participants and their families to revise care plansInstruct participants, their families, and caregivers regarding the disease process, self-care techniques, and prevention strategiesJob requirements include:Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferredUnencumbered California RN LicenseCurrent Basic Life Support (BLS)/First Aide Certification requiredTwo (2) years of nursing experience in a clinical setting with a frail or elderly population preferredMust obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferredWe are seeking Case Manager, RNs that ideally have experience working with a frail and elderly population.
