What You’ll Do As a Clinical Case Manager at Connected Crew, you’ll play a vital role in supporting clients on their journey to wellness. Your responsibilities will include: Assessing client needs and developing individualized care plans. Coordinating services and resources to ensure clients receive the support they need. Monitoring and evaluating client progress to ensure goals are being met. Maintaining accurate and timely documentation of client interactions and case notes. Collaborating with a multidisciplinary team to provide holistic care. Advocating for clients and connecting them with community resources. What We’re Looking For We’re seeking a candidate who is compassionate, organized, and ready to make an impact. To succeed in this role, you’ll need: At least 1 year of experience in case management, social work, or a related field. Strong communication and interpersonal skills. A detail-oriented mindset with excellent organizational abilities. The ability to work both independently and collaboratively in a team setting.
