Job Overview Fairview Range is hiring a Social Worker to join our Oncology Department in Hibbing, MN. The Social Worker will provide professional social services to patients and families receiving care and/or services at Fairview Range Mesaba Clinic. Organize health care services to promote optimal outcomes for individuals utilizing resources, payor sources and clinical data. Assist in meeting the psychosocial needs of patients and to utilize medical care in order to achieve their optimal level of health. The social worker will serve as a patient advocate and coordinator of patient care. They will assist in the identification and utilization of internal and external referral sources to augment or meet patient needs. Services provided will be in accordance with current federal, state, and local standards. Social Services will promote utilization of health care resources. Schedule: This position is full time, working 80 hours per two weeks, 8-hour Day shifts and no weekends. Compensation Disclaimer The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions. - 60+ service lines
- Primary care includes over 20 providers and more than 50 support staff that see patients in Hibbing, Mt. Iron, and Nashwauk.
- Specialty care includes over 35 providers and more than 40 support staff that see patients in Hibbing and Mt. Iron.
- Specialty departments include surgical services, orthopedics, OB/GYN, ENT, allergy, wound care, podiatry, and more.
- Care team includes Physicians, Advanced Practice Providers, RN Care Coordinators, RNs, LPNs, MAs, Patient Access Coordinators
- Hospital-based clinic system
- Provide direct social work services to patients and their families and/or groups of patients through counseling, planning and coordination of services and community resources.
- Social and practical environmental problems including identification of those needing financial assistance, and either assisting or referring to the patient liaison, seeking financial aid, securing a homemaker, arranging for home care, discharge planning including collaboration with community agencies, and referrals to other community agencies for specialized help.
- Determines social assessments of total patient needs.
- Determines the patient's social, psychological, financial, cultural, and family situation.
- Obtains resources from community social, health and welfare agencies.
- Provides consultation to members of clinic and hospital staff, community agencies, and other persons or groups seeking guidance in efforts to solve the problems of patients.
- Participates as a member of the health care team for the purposes of care planning and delivery.
- Advocates for the patient.
- Monitors and reports any and all vulnerable adult or abuse/neglect concerns to the appropriate agency and assists in the investigative phase as necessary.
- Works in conjunction with Utilization review staff to ensure patients' needs and criteria are planned for and met.
- Participates in care conferences with providers, patients and families.
- Completes mental health assessments and crisis evaluations upon request.
- Assists with Advanced Directives as needed.
- Responsible for the complete orientation and maintenance of records of all staff, along with any mandatory education.
- Bachelor of Science in Social Work
- Licensed Social Work current, active Minnesota licensure
- Master of Social Work
- Previous experience in acute and/or primary care setting
