Advanced Practice Provider - Geriatric & Wound Care

Role Overview We are seeking a compassionate, motivated, and highly skilled Physician Assistant (PA) or Nurse Practitioner (NP) to join our dynamic healthcare team. This role is designed for a mobile provider to deliver primary care and wound care services to patients in their homes and Assisted Living Facilities (ALFs) within the Dallas-Fort Worth area. It is a flexible, independent position with the opportunity to work either part‑time or full‑time, focusing on geriatric patients who often have complex comorbidities and face challenges leaving their home for medical care. Key Responsibilities Deliver primary care and wound care services to patients in their homes and ALFs. Operate as an independent mobile provider, working part‑time or full‑time. Focus on geriatric patients with complex comorbidities. Qualifications and Requirements Active Physician Assistant (PA) or Nurse Practitioner (NP) licensure in the state of Texas. Certification as a PA or NP from an accredited program. Previous experience in geriatrics, primary care, or wound care is preferred. Comfortable working independently with minimal supervision. Strong clinical judgment, problem‑solving skills, and attention to detail. Experience with wound care, including debridement and advanced skin allograft products, preferred but not required. Willingness to learn and develop wound care skills is a must. Ability to build rapport with patients and their families, demonstrating empathy and patience. Must have reliable transportation and be willing to travel to patient locations.
