About the Role:

As an LPN on our evening shift, you will be responsible for providing direct patient care to our residents, including administering medications, performing vital signs, and documenting patient information. You will also collaborate with our interdisciplinary team to develop and implement individualized care plans, ensuring that each resident receives the best possible care to meet their unique needs. In addition, you will maintain accurate and detailed records, communicate effectively with patients, families, and healthcare professionals, and contribute to a positive and supportive environment.

Responsibilities:

• Provide direct patient care to residents, including administering medications, performing vital signs, and documenting patient information

• Develop and implement individualized care plans in collaboration with the interdisciplinary team
