Job Overview The Care Manager/Designated Care Manager is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long‑term care and reminiscence neighborhoods. The role involves hands‑on care, physical and emotional support, and safe, comfortable home‑like environments in accordance with federal, state, and local laws, regulations, and Sunrise Senior Living standards, policies, and procedures. Responsibilities Review, notate, and initial daily logs to document resident information. Participate in the development and monthly updates of Individualized Service Plans (ISPs). Manage a designated group of residents during a shift, including location checks and physical assessments. Observe, report, and document changes in resident condition (skin, behavior, weight, diet, activities). Attend daily Cross Over meetings and notify supervisors of increased care needs. Respond promptly to security system and resident call bells, resetting call bells as needed. Greet guests, family members, residents, and team members; answer phone calls courteously. Assist with continence management and sanitation of resident suites. Communicate with families as a resource when appropriate. Help residents maintain independence, physical safety, and dignity in accordance with Resident Rights and Sunrise Principles of Service. Participate, lead, and assist residents with activities of daily living (ADLs) and Invite, Encourage, Assist (IEA) activities per ISP. Engage residents in life skills and enrichment activities, including reminiscence programs. Maintain a warm, personal, and calming environment and follow established safety regulations. Practice standard care precautions for cleanliness, hygiene, and health. Host and engage residents in daily activities. Partner with the community team on compliance with occupational health and safety regulations. Use personal protective equipment, fire extinguishers, safety data sheets, and lockout/tagout procedures. Report accidents/incidents and unsafe conditions immediately. Ensure proper handling and disposal of blood‑borne pathogens, bodily fluids, and hazardous materials. Store oxygen tanks safely and monitor levels. Maintain and clean resident rooms, including bed making, trash removal, and housekeeping tasks. Wash resident laundry, dining linens, and napkins as required. Serve meals, promote pleasant dining experiences, and follow resident diets. Assist with dining room set‑up and clean‑up, pre‑meal meetings, and safe food handling. Specific to Reminiscence Neighborhood: IEA residents in life skills, attend afternoon social, incorporate “Creating Pleasant Days,” blend multi‑sensory experiences, and participate in letters home. Qualifications High School diploma or GED; may be required per state regulations. CPR and First Aid certification as required by state regulations. Minimum age 18. Previous experience working with seniors preferred. Desire to serve and care for seniors. Ability to make choices and decisions in the resident’s best interest.
