- Conduct thorough evaluations for patients across inpatient and outpatient settings, crafting tailored treatment plans to meet individual needs.
- Implement physical therapy interventions emphasizing mobility, strength, balance, and overall functional improvement.
- Collaborate closely with an interdisciplinary team, including nurses, physicians, case managers, and other rehabilitation specialists to enhance patient outcomes.
- Engage patients and their families in discussions about achievable therapeutic goals, discharge planning, and prescribed home exercise protocols.
- Maintain meticulous documentation of patient evaluations, treatment sessions, and progress in compliance with facility and regulatory standards.
- Take an active role in care team rounds and interdisciplinary conferences to ensure seamless and coordinated care for each patient.
- Doctorate in Physical Therapy (DPT) or equivalent credential from a nationally accredited institution.
- Valid state licensure as a Physical Therapist
- CPR certification preferred
