Signify Health, part of CVS Health, is seeking a Full Time Clinician (NP or PA) to provide In‑Home Health Evaluations, engaging with people in their own homes and helping bridge gaps in care. Each visit lasts about 45 minutes and includes medication and medical history review, a physical exam, and any indicated diagnostic tests by the health plan. You will use an iPad with a streamlined workflow. Travel locally within licensed states and conduct virtual visits as needed. #J-18808-Ljbffr
