Physical Therapist (Home Health)

Role Overview Provides physical therapy services to restore and enhance patients' physical, social, and mental functions within a home health or hospice setting. Collaborates as a co-leader in interdisciplinary teams to develop and implement individualized treatment plans that meet quality standards and departmental goals. Key Responsibilities Conducts comprehensive evaluations and develops tailored treatment protocols. Implements therapy interventions using appropriate techniques and tools. Prepares and communicates personalized care and discharge plans. Maintains accurate clinical documentation within designated deadlines. Supervises assigned therapy assistants and performs additional duties as needed. Qualifications & Skills Doctor of Physical Therapy (DPT) degree or foreign equivalent; licensed in applicable states. Valid state licensure and CPR/BLS certification. Valid driver’s license and minimum age of 21.
