RN Case Manager | Hospice & Home Care SpecialistTake charge of developing, implementing, and managing personalized patient care plans in a compassionate hospice and home care setting. Collaborate closely with physicians and interdisciplinary teams to optimize patient outcomes across all age groups, from newborns to the elderly. Engage actively in delivering exceptional customer service and nurturing respectful relationships with patients, families, and colleagues.Essential FunctionsCreate individualized care plans based on medical and nursing diagnoses and patient goals.Provide skilled nursing services aligned with the care plan.Complete accurate OASIS assessments as required.Coordinate with physicians, patients, families, and other disciplines to adjust care plans as needed.Supervise Home Health Aide services according to policy.Participate in on-call rotations to ensure 24/7 nursing availability for hospice, homecare, and private duty patients, including scheduled visits and admissions.Maintain timely and precise documentation following agency guidelines.Foster positive working relationships with patients, families, healthcare providers, and referral sources.Encourage patient independence by involving them and their families in care discussions.Observe and report on patients' physical, mental, emotional, and spiritual conditions, collaborating with the care team as appropriate.Adhere strictly to HIPAA regulations protecting patient information.Minimum Knowledge and SkillsProficiency with MS Office and keyboarding skills.Strong communication abilities.Effective multitasking and prioritization.Basic nursing principles application.Preferred Knowledge and SkillsClinical nursing skills.Experience with Home Health OASIS Data Set.Required Behavioral SkillsIntegrity: Demonstrate honesty and ethical behavior, inspiring others to uphold organizational values.Compassion: Show empathy and provide excellent customer service to all individuals encountered.Accountability: Take ownership of projects and adapt flexibly to changing priorities.Respect: Collaborate effectively with diverse populations and maintain a positive service orientation.Excellence: Exhibit strong communication and leadership skills, guiding teams and facilitating process improvements.Education and ExperienceGraduate of an accredited nursing program.Several years of clinical nursing experience.Preferred: Bachelor of Science in Nursing (BSN).Preferred: Previous hospice and/or home health experience.Preferred: Certification as a Hospice and Palliative Care Nurse.Licenses and CertificationsCurrent Registered Nurse license in the relevant state.BLS certification required within three months of hire.Valid driver's license and auto insurance meeting state requirements.This role is based in a regional healthcare setting serving communities in the Central United States.
