LPN Charge Nurse

The LPN ChargeNurse is responsible for clinical and administrative tasks in a single homesetting for one individual requiring 24/7 skilled care. This young adult isengaged in her community and leads a full lifeâ€"as long as her medical needs aremanaged by a skilled nurse. The Charge Nurse is responsible for coordinatingcare with the LPN team at the home, physicians and other community healthcareproviders, and non-clinical Georgia Options staff who assist the individual tolive as independently as possible.

*This is a Fulltime Monday-Friday Day shift position*

ESSENTIAL FUNCTION AND RESPONSIBILITIES

1. Carry out nursing care according to the careplan

2. Assess for and identify potential healthproblems, report changes in condition

3. Complete and maintain electronicdocumentation of health status

4. Manage, maintain, and administer medicationsand G/J tube feeding

5. Manage medical care with other providers,including staff LPNs

6. Manageequipment, medical supplies, and other specialized needs

7. Advocate forindividual as necessary

8. Serve asliaison for family and Georgia Options

9. Assist Direct Support Professionals toprovide community living support and non-skilled personal care
