Physical Therapist (PT) In-House

If you've worked in SNF therapy under a contract model and felt like the company's priorities didn't always match your patients' needs this role is intentionally different.

We are an in-house, family-owned therapy organization not a contract therapy company. Our therapy teams are clinician-led and clinician-supported, so you can focus on what you were trained to do delivering meaningful, patient-centered care that changes lives.

What You'll Do

• Evaluate residents and develop individualized treatment plans in collaboration with residents, families, and the interdisciplinary care team
• Provide physical therapy services focused on restoring mobility, strength, function, and independence
• Conduct screenings and identify residents who would benefit from intervention
• Supervise and collaborate with PTA staff as applicable
• Initiate referrals when appropriate in accordance with department procedures
• Document clearly and efficiently in support of quality outcomes

What We're Looking For

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
• Current PT license or license-eligible
• Commitment to patient-first, outcomes-driven care
• Team-oriented mindset with strong communication skills
• New graduates welcome mentorship provided from day one
