PT - Physical Therapist

Outpatient Physical Therapist

This is a 13-week contract position.

5x8.0 schedule, starting August.

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans based on the patients' needs, goals, and physician directives. This role involves delegating and directing discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs to aid in effective community reintegration. The therapist will serve as an educator for patients, families, coworkers, and students, while upholding the organization's Mission Statement and Core Values.

Responsibilities:

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans focused on progressing patients toward discharge.
• Document medical records clearly, concisely, and accurately in compliance with various guidelines.
• Ensure care is provided under signed, valid physician orders and insurance approval.
• Participate in student programs, providing a positive learning environment and evaluations.
• Engage in professional development to enhance skills and meet organizational objectives.
• Adhere to discipline-specific State Practice Acts and maintain professional licensure in good standing.
• Understand and follow organizational performance improvement initiatives.

Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an Accredited School.
• 1+ year of outpatient clinical experience.
• CPR Certification.
• Active and applicable state license for practice.
• Prior successful assignments are highly desired.
