Requirements

• Current state license as a Registered Nurse - Required
• Homecare Homebase Software Experience - Required
• OASIS Experience - Required
• 1-2 years experience as a Registered Nurse in a clinical care setting - Required
• 2+ years Home health experience - Required
• Proof of current CPR certification - Required
• Current driver's license - Required
• Must have reliable transportation and insurance - Required
• CHPN certification - Preferred
• Bachelor's of Science in Nursing - Preferred

Responsibilities

• Assess home health and/or hospice patients to identify their needs.
• Implement, develop, document and evaluate the Plan of Care (POC).
• Educate patients and/or their families regarding care treatments and safety.
• Supervise and provide clinical direction to home health aides.
• Utilize appropriate pain management techniques and educate on pain management.
• Provide effective communication and serve as a liaison among care team members.
