RN Care Manager: Chronic Disease Navigator

Community Health Choice, Inc. in Houston, TX seeks a Care Manager to oversee members with chronic disease and complex needs. You will coordinate care, assess and stratify risk, and partner with providers to implement care plans. This role supports utilization review, member services and quality improvement teams.
