Job Summary As a Registered Nurse, you will oversee patient care using a professional practice model. Responsibilities include patient assessment, recognizing health issues, creating care plans, and assessing patient responses. You will secure patient travel, stay knowledgeable about patient needs, prevent issues, follow the Texas Nursing Practice Act, and delegate tasks based on condition and team competencies. Shift: FT/Days 7a-7p, Three 12hr shifts per week, rotating weekends. Essential Functions of the Role Conduct detailed clinical assessments and prioritize patient and family needs. Create, implement, and evaluate care plans, adjusting plans to reach desired outcomes. Build relationships, manage discomfort, and maintain professionalism in therapeutic connections with patients and families. Create care plans that reflect values to aid in resolving issues. Educate patients, families, nurses, and community members, empowering them with knowledge for better healthcare choices and assessing learning needs regularly. Collaborate with colleagues and community to create a care plan; delegate tasks carefully and follow up promptly. Use problem‑solving strategies with patients, families, and staff, recognizing limitations and prioritizing safety, effectiveness, and efficiency in patient care planning and delivery. Achieve goals through teamwork, recognize contributions, and commit to growth. Contribute to peers’ development and demonstrate community service commitment.
