RN Case Coordinator

Hartford Healthcare is seeking an RN Case Coordinator to join our Healthcare & Medical Services team. In this role, you will coordinate care across the continuum, develop and monitor patient-centered care plans, and facilitate safe transitions between inpatient, outpatient, and community settings. You will collaborate closely with physicians, nurses, and interdisciplinary teams to optimize resource utilization, reduce readmissions, and support families through education and advocacy.
