RN Case Manager: Care Coordination & Discharge Planning

Trinity Health in Des Moines, IA is seeking an RN Case Manager to support MercyOne Care Coordination with night shift responsibilities. You will partner with physicians, insurers, and care teams to ensure patients receive appropriate care across the continuum. You will review admission status, coordinate services, assist with discharge planning, and connect patients to resources, driving patient-centered care and safe transitions. #J-18808-Ljbffr
