Physical Therapist (PT) - 13-Week Contract

This is a 13-week contract position with guaranteed hours.

Join our team as a Physical Therapist (PT) and provide top-notch care to patients in a post-acute setting. Your role will involve evaluating patients, developing tailored treatment plans, and implementing therapeutic interventions to promote recovery and optimize physical function.

Responsibilities:

• Conduct initial evaluations and assessments of patients' physical abilities and needs.
• Develop and implement individualized treatment plans to address specific patient goals.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and their families on treatment options and exercises.
• Document all patient interactions and treatment outcomes according to facility guidelines.

Required Experience / Certifications / Licensure:

• Current state license as a Physical Therapist.
• Experience in a post-acute or rehabilitation setting preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
