Denver, CO metro area - Full-Time / Hourly

We are looking for an Admissions Coordinator who wants to help enhance the quality of life of people with intellectual or developmental disabilities who are receiving or seeking to receive services with PPCH.

What You'll Do

As an Admissions Coordinator, you will be responsible for ensuring a seamless admission experience for families, guardians, and people with disabilities as they transition to or begin to receive services from PPCH. You'll help support people receiving services in creating the lives they desire by facilitating successful partnerships and services. You will also act as liaison between our members receiving services, Host Home Providers, family members, and team members to ensure the pursuit of the member's goals and choices. In addition, you'll ensure that services and support are provided in a way that respects and acknowledges an individual's self-expression, self-worth, self-reliance, and effective decision-making.

Some of what you will do is:

  • Build and maintain relationships with referral sources, families, guardians, and people with disabilities looking to receive services in the community and respond to inquiries, comments, and complaints promptly and professionally
  • Continually and consistently evaluate and assess programs, and make recommendations regarding improvements to services, supports, and customer satisfaction
  • Assist with the onboarding and training of newly contracted providers and ensure that training, resources, and work environment are provided to support our members in services in pursuing their goals, activities of interest, and social interactions
  • Attend team meeting and ensure implementation of the person-centered service plan and applicable rights modifications for our members in services
  • Complete inspections of the living environments for members in residential services from PPCH to make sure standards are met
  • Ensure bookkeeping and recordkeeping for caseload meets standards of PPCH and State and Federal agencies and also know and understand PPCH and other regulating agencies' policies, procedures, rules, regulations, and guidelines

What You'll Need

  • High school diploma or equivalent is required (Bachelor's degree is preferred)
  • Successful completion of the required training provided by PPCH
  • 2+ years of work experience in the field of social services or human services, serving children, teens or adults with intellectual or developmental disabilities (I/DD) is required
  • Strong computer skills with proficiency using Microsoft Office suite or similar software is required
  • Excellent written and verbal communication skills, including effective listening skills, even when non-traditional communication is used is important
  • Ability to lead, influence, direct and motivate team members, families, and contractors in order to provide quality services, and onboard individuals seeking support quickly and collaboratively in their preferred setting is key
  • Strong organizational skills, effective and creative negotiation skills, the ability to prioritize well, think critically and problem-solve is necessary
  • Must be dependable, trustworthy, and self-motivated with the ability to work independently with little or no direct supervision, once trained
  • Must have a valid driver's license and the ability to meet PPCH's driving requirements

Important - Physical Requirements and Working Conditions

  • Work may be performed in a typical office environment, in a private home setting, or out in the community, and may be performed in person, over the phone, or virtually
  • Frequent lifting, moving, and/or carrying of approximately 30 40 pounds
  • May be required to bend, stoop, squat, or climb as it pertains to assessing an environment
  • Work may include driving throughout the Denver metro area on a regular basis

You'll Get

Hourly Pay Range: $26 - $30
