The Case Manager collaborates with other healthcare team members to coordinate and facilitate the effective delivery of quality services to Calvary patients. The Case Manager is knowledgeable about and responsible for treatment plans, utilization review, discharge planning, psychoeducation groups, and case management. Qualifications Case Manager Job Requirements: Bachelors degree required, Master's degree preferred. Minimum of 2 years of experience in case management related work. Valid Fingerprint Clearance Card Completion of BLS and First Aid Trainingwithin 30 days of employment.
