Location: Lebanon, New Hampshire / White River Junction, Vermont area

Pay Rate: $65 to $70/hr

Contract: 13 weeks

Schedule: Day Shift

Guaranteed Hours: 36 hours per week

Experience: Minimum 2 years required


About the Role


We are seeking an experienced Hospice Registered Nurse Case Manager for a 13-week contract opportunity serving the Lebanon, NH and White River Junction, VT areas. This role focuses on hospice case management, comfort care, pain management, care coordination, and support for patients and families during end-of-life care.


Responsibilities
• Develop and update individualized hospice care plans with physicians, patients, and families
• Provide ongoing clinical, functional, and emotional assessments
• Coordinate patient care through interdisciplinary communication and case conferences
• Support pain management, comfort care, and end-of-life symptom management
• Document evaluations, patient response, and care progress accurately using EPIC


Requirements
• active registered nurse license in vermont and new hampshire or compact license required
• graduate of an accredited school of nursing required
• bls/bcls certification required
• minimum 2 years of nursing experience required
• hospice, medical/surgical, oncology, home health, or case management experience preferred
