As an LPN , you will provide health screening, coordination of health referrals, and work in close collaboration with prescribers and staff in the Adult and Child/Family Clinics, PROS and Crisis programs under the direction of a Registered Nurse or other authorized health care provider.

Work Schedule: Monday through Friday - 7:30 to 3pm

What your day might look like:

• Dispenses medication as prescribed by the Psychiatrist/Nurse Practitioner
• Complete weekly checks in the Medication Administration Records
• Provides crisis intervention services as needed, including offsite in the community
• Forwards reports, labs, notes to PCPs as requested
• Facilitate Health and Wellness groups
• Conducts urine drug screenings in accordance with agency policies
• Utilizes electronic health record system for all patient documentation
• Ensures the security and inventory of all medications and nursing supplies
• Attends department meetings and trainings as required
• Performs other duties as assigned within context of job description

Your skills and qualifications:

• NYS Department of Education Licensed Practical Nurse
• Excellent engagement, organizational and communication skills Detail-oriented
• Ability to work with electronic database management systems
