We are growing and looking for a compassionate, dependable, and skilled PRN Licensed Practical Nurse (LPN) to join our Home Health team. What You’ll Do As a PRN Licensed Practical Nurse (LPN), you’ll play an important role in supporting the health, safety, and independence of our residents within their apartment homes throughout the community. Your responsibilities may include: Medication management and administration Performing treatments and nursing procedures as ordered Monitoring resident conditions and completing assessments Responding appropriately to emergencies and resident needs Communicating changes in condition to the nursing team and physicians Assisting with activities of daily living (ADLs) when needed Supporting and guiding Home Health Aides and caregivers Maintaining accurate clinical documentation and records Promoting infection control and safety practices Creating a calm, respectful, and positive environment for residents What Makes This Role Special Work in a supportive, team-oriented environment Build meaningful relationships with residents and families Make a direct impact on residents’ quality of life and independence Be part of a mission-driven organization that values compassion and teamwork Opportunities to grow your healthcare skills and experience What We’re Looking For A compassionate, patient, and dependable individual Strong communication and teamwork skills Someone who treats every resident with dignity, kindness, and respect Ability to work independently while also collaborating with the care team A positive, solution-oriented attitude with strong critical thinking skills Physical & Work Environment This role includes standing, walking, assisting residents with mobility and daily activities, and occasional lifting up to 50 pounds. Requirements Current and valid Licensed Practical Nurse (LPN) license in the State of Indiana Valid Indiana driver’s license and reliable transportation High School Diploma or equivalent Ability to maintain confidentiality and follow HIPAA guidelines Strong communication, organizational, and clinical decision-making skills CPR certification (or willingness to obtain — we provide certification if needed!)
