RN Care Manager for LTSS & Care Coordination

Molina Healthcare is seeking an RN Care Manager to support care coordination for LTSS members in New York. You will collaborate with a multidisciplinary team to develop and monitor care plans, address barriers, and ensure services are delivered timely and effectively. Responsibilities include comprehensive member assessments, waiver enrollment management, and coordinating with providers, caregivers, and the member support network.
