Trauma-Informed Case Manager - Community & Home Care
Crossroads Health
Crossroads Health in Northeast Ohio is seeking a dedicated Case Manager for North Coast House. You will provide trauma-informed case management services in residential and community settings, linking clients to resources and supporting transition after discharge. Responsibilities include home visits, group services, crisis intervention as needed, and meeting monthly productivity metrics. A High School Diploma or higher is required, with preferred Associate’s or Bachelor’s in Human Services or #J-18808-Ljbffr Crossroads Health
