CHILDREN SERVICES - FAPT CASE MANAGER

SBH is seeking a qualified applicant for its FAPT case management in the children services department, located in South Boston, VA. The FAPT Case Manager will maintain an assigned caseload while also serving as the primary representative for the Family Assessment and Planning Team (FAPT) in Halifax County. The Case Manager will work collaboratively with youth, families, service providers, schools, Department of Social Services, court personnel, and other community partners to coordinate and support individualized services. The FAPT Case Manager for Halifax will balance direct case management responsibilities with the coordination and representation of FAPT services for the locality. This position requires strong organizational, communication, documentation, and collaboration skills, as well as the ability to coordinate services for youth with complex needs, including youth receiving residential services. Works extended hours one day a week until 7:00 pm. Responsibilities FAPT Representation and Coordination Serve as the primary FAPT representative for Halifax County. Attend and actively participate in scheduled FAPT meetings and related staffing meetings. Prepare cases for presentation to FAPT by ensuring all required assessments, documentation, consents, service recommendations, and supporting information are completed and submitted within required timeframes. Coordinate with families, providers, and community partners to gather information necessary for FAPT review and service planning. Follow up on FAPT recommendations and ensure approved services and supports are coordinated appropriately. Case Management Responsibilities Maintain an assigned caseload and provide ongoing case management services in accordance with agency, CSA, Medicaid, and applicable regulatory requirements. Complete and maintain Individualized Family Service Plans (IFSPs), assessments, consents, releases of information, quarterly reviews, and other required documentation. Ensure that all documentation is completed accurately, timely, and in accordance with agency policies and funding requirements. Monitor the progress of youth and families toward identified goals and update service plans as needed. Coordinate and collaborate with treatment providers and other involved professionals to ensure continuity of care. Residential Services Monitoring Conduct required visits with all Halifax youth placed in residential services. Maintain regular contact with residential providers, youth, families, and other members of the treatment team. Monitor each youth’s progress, treatment needs, discharge planning, and transition back into the community. Participate in treatment team meetings, discharge planning meetings, and other meetings related to the youth’s care as needed. Ensure appropriate documentation of all contacts, visits, service coordination activities, and progress. CANS Certification and Assessment Responsibilities Successfully obtain and maintain certification in the Child and Adolescent Needs and Strengths (CANS) assessment. Complete CANS assessments and reassessments as required. Utilize assessment results to assist with identifying the strengths, needs, and service priorities of youth and families. Maintain compliance with all CANS certification and recertification requirements. Cross-Locality and Backup Responsibilities Serve as a backup representative for other localities when needed. Attend FAPT meetings in other assigned localities when the primary representative is unavailable. Provide support with case preparation, service coordination, documentation, and other FAPT-related responsibilities as assigned. Work collaboratively with other FAPT Case Managers to ensure continuity of services and adequate coverage across localities. Additional Expectations Maintain effective communication with supervisors and provide updates regarding caseload concerns, service barriers, residential placements, and high-risk situations. Participate in required supervision, staff meetings, training, and professional development activities. Develop and maintain positive working relationships with families, community partners, service providers, and other stakeholders. Perform other duties as assigned to support the needs of the Children and Family Services Department. Qualifications Bachelor’s degree in human services, social work, psychology or any human services related field required. QMHP-C preferred, ability to obtain QMHP-C required. Experience in the provision & coordination of services to the mentally disabled/children services at a local community level (to include assessment, evaluation & planning of services, casework documentation, as well as utilization of available community resources for the mentally disabled). Must meet all criminal background and drug screening requirements. Must be able to work independently. Must possess a valid driver’s license and safe driving record.
