Home Health Physical Therapist (PT) Flexible Schedule
Concierge Home Care is hiring a Home Health Physical Therapist (PT). If you want more control over your schedule, meaningful patient relationships, and the ability to increase your income based on productivity, this is the role for you.
Coverage Area: Tallahassee, FL (Territories designed to minimize drive time with clustered scheduling)
Compensation & Earning Potential
- Pay-per-visit model: $70+ per visit + Medicare differential pay
- Average full-time earnings: $120,000+
- Quarterly bonus opportunities

Your Role as a Home Health Physical Therapist
- Evaluate patients and develop individualized treatment plans
- Provide therapeutic exercises and mobility training
- Educate patients and caregivers on safety and recovery
- Collaborate with physicians and interdisciplinary teams
- Supervise Physical Therapist Assistants (PTAs)
- Complete documentation using streamlined technology

Qualifications
- Active, unencumbered Florida Physical Therapist license
- Valid driver's license, reliable transportation, and auto insurance
