Value-Based Care RN Case Manager - Dialysis & CKD Advocate
U.S. Renal Care
U.S. Renal Care in Texas is seeking a Value-Based Care Nurse Case Manager to support chronic kidney disease patients through education, care coordination, and comprehensive assistance with medical history, medications, and lifestyle changes. This role may involve telephonic or in-person visits at USRC Dialysis Clinics or physician offices. The successful candidate will collaborate with nephrologists and the dialysis team, drive key patient outcomes metrics, and be based full-time in Texas with #J-18808-Ljbffr U.S. Renal Care
