RN Care Manager Lead — Chronic Care Coordination

Case Management Society of America (CMSA) ® is seeking a Care Manager Team Leader in Houston, Texas. In this role, you will manage CHC members with chronic diseases, providing comprehensive evaluation and resource coordination. The ideal candidate should hold an RN license in Texas and have five years of clinical experience, with a preference for one year in case management. The position emphasizes strong analytical and communication skills, and a CCM is required or must be obtained within 18 months.
