Rate of Pay: $172,515 annually + DOE

Summary

The Advanced Practice Provider (APP) Lead-Mobile Health Services serves in a leadership position responsible for supporting the Primary Care Medical Director, Primary Care Operational Director and Director of Community Health in overseeing mobile health services clinical operations, ensuring high quality patient care and maintaining compliance with regulatory standards. This role involves supporting mobile health providers, assisting with program development and contributing to clinical policy and protocol implementation as well as translating organizational and community health priorities into sustainable healthcare services that address barriers to care and improve health outcomes. The APP Lead-Mobile Health Services will also provide direct patient primary care.

Essential Duties and Responsibilities

• Develops and leads implementation of strategic plans and clinical frameworks for Mobile Health Services in collaboration with the Director of Community Health and organizational leadership.
• Develops recommendations regarding service offerings, deployment strategies, geographic coverage, staffing models.
• Assists with development and execution of business plans and growth strategies for new and existing mobile healthcare services as needed
• Develops and implements clinical workflows, operational standards, policies, and procedures supporting mobile healthcare delivery.
• Ensures patient care and clinical guidelines are followed to promote consistent, high-quality, safe, and cost-effective care.
• Establishes and monitors quality, safety, access, productivity, and patient experience metrics.
• Ensures compliance with Rural Health Clinic certification requirements, billing standards, documentation requirements, and applicable federal and state regulations.
• Develops staffing and care delivery models that support continuity, access, and sustainable Rural Health Clinic operations.
• Explores innovative care delivery models designed to address transportation barriers, workforce limitations, and healthcare access disparities.
• Assists in recruitment, development, supervision, evaluation, and retention of clinical and support staff.
• Supports provider recruitment, onboarding, workforce planning
• Builds and supports high-performing interdisciplinary teams focused on service excellence and community impact.
• Assists community health in representing Mobile Health Services to internal and external stakeholders.
• Develops collaborative relationships with healthcare providers, community organizations, educational institutions, employers, public agencies, and other strategic partners.
• Acts as liaison between Mobile Health Services, leadership, providers, staff, patients, and community partners.
• Patient care: demonstrates clinical competence to provide adult or family primary care. Provides primary care medical services for assigned patients including preventive physical examinations, evaluation and treatment of injuries and illnesses, chronic disease management and referrals to appropriate specialties or facilities.
• Orders, performs, interprets diagnostic tests as appropriate
• Prescribes medication, therapy and other specialized medical care per standard of care
• Performs appropriate medical procedures within scope of practice as defined by licensing and the health system’s credentialing and privileging processes
• Documents within the electronic medical record system to facilitate ongoing quality of care in a timely manner
• Able to distinguish between normal and abnormal findings
• Collaborates with and consults the supervising physician when appropriate and required by policy and completes chart review as required by health system policies.
• Escalates care appropriately when cases exceed training or expertise.
• Provides patient care for same day and urgent visits
• Adheres to all relevant policies, procedures and practice guidelines.
• Supports appropriate billing for professional services through timely documentation and maintaining adequate knowledge of the practice’s billing policies and procedures

Supervisory Responsibilities
Carries out supervisory responsibility in accordance with the organization’s policies and applicable laws. Responsibilities include interviewing, hiring, training, assigning, coaching, counseling, and disciplining employees; administering scheduling systems; communicating job expectations; planning, monitoring, appraising, and reviewing job contributions; enforcing policies and procedures.

Education and Experience
Nurse Practitioner: Bachelor’s degree (B.S.) from four-year college or university; Graduation from a National League of Nursing (NLN), Commission on Collegiate Nursing Education (CCNE) or equivalent accredited school of nursing. The nurse shall have completed a nursing education program that included clinical experiences with actual patients under the supervision of qualified faculty. Must be certified by the American Academy of Nurse Practitioners (AANP) or the American Nurses credentialing Center (ANCC).

Physician Assistant: Completion of a Physician Assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant.

Licenses, Certifications
Nurse Practitioner: 1) Current Advance Practice RN (APRN) licensure to practice in California (and/or NV, depending upon the position being sought); 2) Current ANCC (American Nurses Credentialing Center) or AANP (American Academy of Nurse Practitioners) certification required. Current PNCB (Pediatric Nursing Certification Board) or current ANCC is required if requesting to work in pediatrics. 3) Current BLS (Basic Life Support) and must maintain certification at all times. 4) Must have a Collaborative Service Agreement (CA) or evidence of Supervising Physician Agreement (NV State Medical Board), as applicable, with a designated supervising physician member of the Hospital’s medical staff (upon hiring, manager will initiate). 5) Current, unrestricted DEA certificate in CA and/or NV, as applicable (Schedules II-V). Nevada Pharmacy Board certificate, if applying to work in NV.; 6) Ability to participate in a federally funded program (Medicare and Medicaid).

Physician Assistant: 1) Current CA and/or NV State license in good standing; 2) Current NCCPA (National Commission on Certification of Physician Assistants) certified; 3) Current BLS (Basic Life Support) and must maintain certification at all times; 4) Must have a Collaborative Agreement (CA) or evidence of Supervising Physician Agreement (NV State Medical Board), as applicable, with a designated supervising physician member of the Hospital’s medical staff (upon hiring, manager will initiate). 5) Current, unrestricted DEA certificate in CA and/or NV, as applicable (Schedules II-V). Nevada Pharmacy Board certificate, if applying to work in NV; 6) Ability to participate in a federally funded program (Medicare and Medicaid); 7) Physician Assistants must complete an educational course in controlled substances that meets the standards of practice by TFHD and the State of California.

Valid Driver's License in good standing.

Other Experience/Qualifications:

Required:

• Five (5) years or more leadership, management, operational, or program development experience within a hospital system, ambulatory care setting, physician group, community health organization, or rural healthcare environment.

• Five (5) years or more of progressively responsible clinical, operational leadership, or program development
• Knowledge of ambulatory care operations, healthcare access initiatives, and interdisciplinary care delivery.
• Understanding of Rural Health Clinic operations, regulations, reimbursement models, and compliance requirements.
• Strong communication, leadership, project management, organizational, and relationship building skills.
