*What is the role of Case Manager?*

We are searching for a Skilled Nurse to support and manage our patients in the Greater Boston area. You must be known to be warm, extremely focused, professional, compassionate, communicative, talented, experienced, and reliable.

*Your day will include*:

* Providing skilled nursing care to adult patients in a home setting
* Implement, develop, and document the *plan of care* to ensure quality and continuity of care for each patient with *admissions, recerts and resumptions of care and supervisory visits*.
* Provide effective communication to patient/family, team members, and other caregivers involved
* Implement plan of care and treatment using *sound judgment* and *critical thinking*
* Provide accurate, detailed, and *timely documentation* consistent with the plan of care and the nursing process
* Administer prescribed medications and treatments, per MD order and Participate in interdisciplinary staff meetings to exchange ideas and information as it relates to the care and quality initiatives.

_*Qualifications*_:

* Must have a criminal background check
* Current Registered Nurse in the state of MA
* *Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.*
* Knowledge of HIPAA regulations
* Current CPR accredited by American Heart Association and/or American Red Cross (certificate must be healthcare provider or healthcare professional).
* Valid MA Drivers license

Pay: $35.00 - $40.00 per hour
