Home Health Physical TherapistThe Home Health Physical Therapist plays a critical role in delivering personalized rehabilitative care to patients within their own homes, promoting recovery, mobility, and independence. This position requires assessing patients' physical conditions, developing tailored treatment plans, and implementing therapeutic interventions to address a variety of musculoskeletal and neurological conditions. The therapist collaborates closely with patients, families, and interdisciplinary healthcare teams to ensure comprehensive care and optimal outcomes. By providing education and support, the therapist empowers patients to manage their health and prevent further injury.
