Physical Therapist (Outpatient) Mesquite, Texas

The organization is seeking an Outpatient Physical Therapist to evaluate assigned patients, develop and implement individualized treatment plans, and support progress toward discharge goals. The role includes timely, accurate medical record documentation, adherence to valid physician orders and insurance approval requirements, and participation in ongoing clinical and professional development activities. Candidates are required to bring their license on the first day.

Job Details

• Schedule: Days, 8 hours per shift, with a minimum of 40 hours per week.
• Required Qualifications: Degree in Physical Therapy from an accredited school; CPR Certification; active/applicable physical therapy license for the state where work will be performed; 1+ year of outpatient clinical experience.
• Preferred Qualifications: Experience from a prior assignment with the organization (highly desired); experience from a prior assignment with the organization previously (nice to have).
• Responsibilities: Evaluates assigned patients and completes comprehensive assessments; utilizes clinical reasoning to develop treatment plans and goal-setting focused on discharge within an established length of stay; performs clear, concise, accurate, and timely documentation including documentation, goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry using approved formats in accordance with state, federal, and intermediary guidelines and clinic policies; ensures care is provided under signed, valid physician orders and within required insurance approval as indicated; participates in student programming as assigned and appropriate, including providing supervision and learning objectives and completing student evaluations; creates an individualized professional development plan with clinical management; identifies and initiates professional growth through continuing education, self-study, and professional activities; applies learned information to support strategic objectives; adheres to the discipline-specific State Practice Act and other regulatory agencies; maintains professional licensure in good standing and completes state-mandated continuing education requirements; performs in an ethical manner in accordance with discipline-specific professional code of conduct; supports organizational performance improvement methodology by following action plans and providing feedback through staff meetings, focus groups, or direct supervisor.
