Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients in New York, New Jersey, Maryland, DC, Virginia, Oklahoma, Kansas, Pennsylvania, and Georgia. Job Summary The Collaborating Physician will have overall responsibility for the medical component of all activities and conduct annual wellness exams once per year for our patients. They will serve as a medical resource to the interdisciplinary group, complement attending physician care, and may be an employee or work under contract. Responsibilities Complement attending physician care Act as medical resource person to the IDG Assure overall continuity of the hospice medical services Assure that the patient receives appropriate measures to control uncomfortable symptoms Collaborate with the IDG to ensure medical needs of patient are met Provide oversight of the plan of care, medications and treatments Adhere to regulations, standards, and conditions of participation required by Medicare, accrediting bodies, and federal and state statutes governing provision of services Certify that patient meets the medical criteria for hospice admission based upon available diagnostic and prognostic indicators, related diagnoses if any, current subjective and objective findings, medication and treatment orders, information about medical management of any of patient’s conditions unrelated to the terminal illness Re‑certify patients as appropriate for continuation of Medicare Hospice Benefit at appropriate levels of care Serve as medical resource to hospice staff, patients, families, and attending physicians regarding pain and symptom control management Ensure provision of direct medical services to patients either directly or through arrangements, as appropriate, in the absence of the patient’s attending physician Attend IDG conferences Act as primary physician for patients whose referring/attending physicians desire to relinquish that care or are not available, and/or if patient/family chooses the hospice Medical Director to assume the role of Attending Physician for them Document care provided in the patient’s clinical record providing evidence of progression of the end‑stage‑disease process Maintain current knowledge of the latest research and trends in hospice care and pain/symptom management Work in a team approach with the IDG Participate in performance improvement programs, as indicated Provide consultation and education to colleagues and attending physicians related to admission criteria for hospice and palliative care Review and develop protocols for treatment and propose the most current options for interventions Demonstrate knowledge in communications and counseling patient/family in dealing with end‑of‑life issues Participate in resolution of interpersonal conflict and issues of clinical and ethical concern Participate in development and updating of patient care policies and emergency procedures Serve as hospice champion in the community Act as liaison to physicians in the community Job Conditions Position requires deadlines and timelines required by governing and regulatory organizations. It requires minimal lifting of office records and printouts. The primary duties are desk work involving sitting, standing, stooping and walking, and telephone communication. Face‑to‑face visits may be required, entailing moderate physical exertion and car travel. Travel may be required by car or airplane to local, out‑of‑town or state seminars, conferences or meetings. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of patient’s condition and perform and demonstrate patient care. Equipment Operation Utilization of a computer, multi‑line telephone, copy machine, and other office and basic medical equipment. Qualifications Licensed or authorized to work in NY, NJ, MD, VA, GA, FL, OK, KS, TX, PA Must be a Doctor of Medicine (MD) or Doctor of Osteopathy (DO), licensed to practice medicine in all of our practices Certified in Hospice or a related specialty Has experience with end‑of‑life care Maintains controlled substances registration with state and federal authorities Must demonstrate self‑confidence and positive attitude toward self and others and maintain commitment and enthusiasm to goal achievement Must not be excluded from the Medicare program Ability to communicate well, both verbally and in writing Benefits for Full‑time Employees Medical, Dental, Vision and supplementary benefits such as Life Insurance, Short Term and Long Term Disability, Flexible Spending Accounts for Medical and Dependent Care, Accident, Critical Illness, and Hospital Indemnity.
