If you're passionate about patient-centered care, enjoy working independently, and want to join a supportive team that values your dedication, we'd love to meet you!What You'll DoManage and coordinate care for an assigned caseload of hospice patientsComplete comprehensive nursing assessments and routine patient visitsDevelop, implement, and update individualized plans of careProvide skilled nursing care, pain management, and symptom controlMonitor changes in patient condition and communicate with physicians and the interdisciplinary teamEducate patients and families about medications, disease progression, comfort measures, and end-of-life careCoordinate medications, medical equipment, supplies, and other patient needsSupervise and collaborate with LVNs and Hospice Aides as appropriateParticipate in interdisciplinary team (IDT) meetingsComplete accurate and timely documentation in the EMRProvide compassionate guidance and emotional support to patients and their familiesWhat We're Looking ForActive Texas Registered Nurse (RN) license or qualifying Nurse Licensure Compact (NLC) license Previous nursing experience in hospice, home health, skilled nursing, or acute care preferred Hospice RN Case Management experience is a plus!
