RN Care Manager

Position RN Care Manager Location Arlington Heights, IL Hours Full-time, Monday-Friday, 8:00 a.m. - 4:30 p.m., with rotating weekends and holiday coverage Responsibilities Include Conduct comprehensive assessments of patients' medical, social, and psychological needs. Develop and coordinate personalized care plans in collaboration with healthcare providers. Identify barriers to care, facilitate referrals, and coordinate services such as home healthcare and specialist visits. Serve as a liaison between patients, families, providers, and community resources for seamless care delivery. Monitor patient progress, manage transitions of care, and ensure timely treatments. Educate patients and families on diagnoses, treatment options, and self-management strategies. Maintain accurate electronic health records and prepare reports on patient outcomes. Participate in quality improvement initiatives and ensure compliance with healthcare regulations, including HIPAA. Qualifications & Skills Bachelor's Degree in Nursing required; Master's preferred. Minimum of 2 years clinical nursing experience, preferably in acute or hospital settings. Current RN license in Illinois; CCM certification preferred. CPR/BLS certification from the American Heart Association. Strong assessment, communication, collaboration, and organizational skills. #J-18808-Ljbffr Aura Health
